The unglamorous code refactoring that I've done on the Drupal project -- cleaning up the FormAPI, consolidating several different tools into a consistent interface, etc -- is probably highest on the list.
Although it's not as impressive or eye catching as some of the great features, the fact that Drupal is an open source project with hundreds, even thousands of people working on the code means that simplifying and smoothing the developer experience helps a lot of people in small ways every day.
Every client is uniquely different, requiring very customized approaches to their brand's needs. Marketing is always different - always challenging - and when successful, very satisfying.
